Supervision – changes of supervisor and other responsibilities

What you and your supervisor must do if you change your employment and/or supervisor.

Anytime you have a change in supervisor the EWRB must approve that relationship, to ensure that the supervision will be appropriate.

The responsibilities both parties must comply with are below.

    1. Advise us using the webform on our Contact Us page so that we can suspend your Limited Certificate pending further information.
    2. When you get a new job and new supervisor, fill in the first section of the Supervision Declaration form then get your new supervisor to fill in the rest of the form and sign the declaration.
